Traumatic Floating 1st Metacarpal in a 14-Year-Old Boy Managed by Close Reduction and Thumb Spica Immobilization: A Rare Case Report

چکیده
INTRODUCTION Double dislocation of thumb metacarpal (MC) is a rare injury which may be secondarily complicated by growth plate injury in children. The management of floating 1st MC is also controversial since the treatment ranges from simple reduction to complex reconstruction surgeries. It is also important to understand the long-term results of different management strategies (close reduction, K-wire fixation, ligament reconstruction) as any residual stiffness or instability of thumb may result in severe disability of the hand. CASE REPORT A 14-year-old boy with an alleged history of injury to the thumb due to a fall. The postulated mechanism of injury was forced hyperextension of thumb and axial loading of hand in the prone position. On examination, there was prominent bony swelling over the dorsal aspect of carpometacarpal (CMC) and metacarpophalangeal (MCP) joints which was very tender with diffuse swelling over entire thumb. X-ray showed dorsal dislocation of both MCP and CMC joints, without any fracture (bony avulsion) or volar plate avulsion. Treatment was by way of closed reduction performed by axial traction followed by forced flexion at MCP joint with continuous pressure over the dorsal aspect of the joint. The reduction of CMC joint was done by direct pressure over the dorsal aspect and full abduction of thumb. Following reduction, the thumb was immobilized in a thumb spica. CONCLUSION Thus, we conclude it is possible to manage a case of floating 1st MC by closed reduction and immobilization, using proper reduction technique. However, a careful clinical and radiological assessment should be done beforehand for signs of bony injury or ligamentous instability.

Isolated Carpo-Metacarpal Dislocation of the Thumb
INTRODUCTION Isolated dislocations of the trapezio metacarpal joint without associated fracture has been reported very rarely in the literature. There are reports of associated injuries like fracture of the trapezium, dislocation of the metacarpophalangeal joint and fractures of the base of the thumb metacarpal. CASE REPORT Authors report a case of isolated carpometacarpal dislocation of the ...

Isolated traumatic cuboid dislocation is a rare injury of which ten cases has been only reported. The reported case in this article is a 12-year old boy who had felt from height and his cuboid bone was dislocated due to severe inversion and plantarflexion injury to the foot. There was not anyother fracturedislocation in his extremity. Closed reduction was unsuccessful therefore the bone was red...
متن کاملOutcome of Thumb Reconstruction Using the First Dorsal Metacarpal Artery Island Flap
BACKGROUND Reconstruction of complex soft tissue defects of the thumb, with exposure of tendons, joints or bones, has always been a difficult task. We evaluated the functional and esthetic outcomes of 1st dorsal metacarpal artery island flap in reconstruction of post-traumatic soft tissue defects of the thumb. METHODS Between January 2012 and June 2014, fifteen patients with co...
متن کاملFanconi Anemia Concurrent with an Unusual Thumb Polydactyly: A Case Report
This case report presents a case of Fanconi’s Anemia with an unusual thumb polydactyly in a 2-year old boy. The extra thumb had no nail, nail bed and distal phalanx. The extra thumb had no active motion. The duplication of the thumb occurred at the carpometacarpal joint but its morphology did not match with any classification described for thumb polydactyly. Although his thumb polydactyly was ...
متن کاملOpen complete dislocation of trapezium with a vertically split fracture: a case report
Open complete dislocation of the trapezium is an extraordinarily rare injury with only a few cases reported so far in literature. The association of a vertically split fracture makes this injury even rare and hence worth reporting. A 14 year old Kashmiri boy presented to us with a history of massive trauma to the non dominant left hand sustained as a result of a blow from a heavy hammer. The th...
